EMSculpt is one of the most widely adopted non invasive body contouring treatments available today, used by dermatology clinics, medical spas, cosmetic surgery centers, and wellness practices to strengthen abdominal muscles, improve muscle tone, and firm the buttocks. Because the procedure involves electromagnetic energy applied directly to the body, providers need documented consent from each client before treatment begins. That consent should cover more than just a signature, it should confirm that the client understands the procedure, has been screened for contraindications, and is aware of how lifestyle factors can influence results.

This EMSculpt consent form template is designed for aesthetic clinics, med spas, and body contouring practices that want a thorough, professional intake document ready to use with every new client. The form is structured so that the client initials each section individually rather than just signing once at the end, which means they are actively confirming their understanding at every stage, not skimming through and signing blindly. This initialing format also strengthens the provider’s legal position, since it demonstrates that the client engaged with each piece of information separately.

Here’s a closer look at each section and how to fill it out.

Client Information

The form starts with a required-field table for the client’s first name, last name, phone number with area code, email address, and the date. This gives your clinic a verified contact record tied directly to the consent, which is important if you need to follow up about treatment plans, schedule changes, or post-treatment check-ins.

Treatment Overview and Session Details

The first initialed section explains what EMSculpt does, non invasive abdominal toning, muscle strengthening, and buttock firming, so the client has a documented understanding of the procedure before it begins. The section that follows covers session logistics: the recommended number of treatments, four, session length, approximately 30 minutes, and the minimum gap between sessions, two days. These values appear in brackets because they are meant to be adjusted based on your clinic’s protocol. The paragraph also makes clear that completing the full treatment series is necessary for best results and that additional sessions may be recommended depending on individual goals. Both sections require the client’s initials, confirming they understand not just what the treatment is but also what the full commitment looks like.

Pre-Treatment Preparation

Before any EMSculpt session, clients need to arrive hydrated, wearing comfortable clothing that allows flexibility for positioning, and without metallic accessories or electronic devices. This section puts those instructions in writing so the client can’t arrive unprepared and claim they weren’t told. It also protects your staff from having to delay or reschedule sessions due to avoidable issues like tight clothing or forgotten jewelry.

Lifestyle Factors and Treatment Outcomes

This is an important liability section. The client acknowledges that smoking, excessive alcohol consumption, eating disorders, and ongoing medication can all affect how well the treatment works. It also encourages healthy eating to support results. By initialing here, the client accepts that their own habits play a role in the outcome, which protects your clinic if someone completes the full series and is unsatisfied with their results due to factors outside your control.

What to Expect During Treatment

EMSculpt does not require anesthesia, but the client will feel intense muscle contractions during the session. There’s no recovery time, and they can return to daily activities immediately. Having this in writing and initialed matters because the physical sensation can be unexpected for first-time clients. If a client later claims they weren’t warned about the intensity of the contractions, this initialed acknowledgment serves as documentation that they were.

pro tip

Even with this written acknowledgment, having your technician verbally walk the client through what the contractions will feel like before powering on the device significantly reduces first-session anxiety and builds trust.

Medical History Screening

This is the most safety-critical section of the form. A checkbox grid lists contraindications that could make EMSculpt unsafe or inadvisable, including drug pumps, malignant tumors, metallic IUDs, hemorrhagic conditions, heart disorders, recent surgical procedures, pulmonary insufficiency, fever, latex sensitivity, injured or impaired muscles, epilepsy, areas of skin lacking normal sensation, and cardiac pacemakers or implanted electronic devices. A “None of the above” option is included as well. If the client checks any condition, an open text field asks them to provide details. This screening exists because EMSculpt uses high-intensity electromagnetic energy, and certain medical conditions or implanted devices can create serious risks during treatment. Do not shorten or remove this section.

Personal Health and Treatment Goals

The final set of questions asks whether the client has been pregnant, with separate checkboxes for C section and vaginal birth, whether they are satisfied with their core muscle strength, and whether they are satisfied with the shape of their buttocks. The pregnancy question is clinically relevant because delivery method can affect abdominal muscle integrity, particularly diastasis recti, which influences how the provider approaches treatment intensity and sets expectations. The satisfaction questions give the provider a baseline understanding of why the client is seeking treatment, which is useful for tailoring the consultation and tracking progress across sessions. A client signature line at the end finalizes the consent.

This template is available in Word, Google Docs, and PDF formats. The Word and Google Docs versions are fully editable, so you can add your clinic’s logo and adjust placeholder values like session count and duration. The PDF version is ready to print for clinics that prefer paper-based intake.

FAQs

Should every client complete this form, or only first-time patients?

Every new client should complete the full form before their first session. For returning clients starting a new treatment series, have them review and re-sign, since their medical history or lifestyle factors may have changed.

Can this form be used for EMSculpt NEO as well?

EMSculpt NEO combines HIFEM energy with radiofrequency, so the treatment explanation and contraindication list would need updates. You can use this form as a starting point but should revise the device description and add RF-specific contraindications such as heat sensitivity or metal implants near the treatment area.

Should the medical history section be reviewed by a physician before treatment begins?

If your clinic operates under physician oversight, having the completed screening reviewed by a licensed provider before the session adds an important layer of clinical judgment and legal protection, especially for flagged conditions like heart disorders, epilepsy, or implanted devices.
