Patient Details

Medical Care and Absence Period

This section certifies that the patient was under medical care during a specific period and unable to attend work. Enter the patient’s name and the start and end dates of care. This statement confirms a past absence and is typically used as retrospective documentation for HR or payroll records.

Return-to-Work Status

Next, indicate the patient’s current work status by selecting one of the three checkboxes:

  • If the patient is cleared to return to work without limitations, select the first option and enter the expected return date.
  • If they are fit to return but require temporary work restrictions, check the second option. Any relevant limitations can be noted in the “Additional Recommendation / Notes” section.
  • If the patient is not yet ready to return, choose the third option and enter the follow-up date when reassessment is expected.

Additional Recommendation

Use this section if you need to include a short note, such as a follow-up recommendation or a general instruction like “may return to light duty.” Avoid listing specific medical conditions unless the patient requests it or it’s required for workplace reporting. If not needed, you can leave this section blank.

Signature and Verification
